BACKGROUND
Injection of heroin and other opioids is prevalent along Kenya’s coastal region and contributes significantly to HIV transmission, particularly in correctional facilities. Medically Assisted Therapy (MAT), particularly methadone maintenance therapy (MMT), plays a critical role in managing opioid dependence and mitigating associated harms.
OBJECTIVE
This study aimed to evaluate the frequency and procedural dynamics of court-ordered and police requested methadone induction among incarcerated individuals at the Shimo MAT Clinic, and to assess associated health and systemic outcomes.
METHODOLOGY
A descriptive case study approach was employed. Thirty-five case files of individuals referred through court or police processes were reviewed using purposive sampling. Semi-structured interviews were conducted to explore procedural experiences, treatment outcomes, and systemic barriers. Thematic analysis identified four major themes: (1) integration between legal and health systems; (2) treatment adherence and clinical outcomes; (3) patient experiences highlighting improved health, family reintegration, and reduced stigma; and (4) challenges such as financial constraints, stigma, and legal inefficiencies.
RESULTS
Of the 35 cases reviewed, 25 involved court-mandated referrals, while 10 were initiated by police. The majority of the respondents (96%) were males aged 25–49 years. Court-mandated clients demonstrated levels of adherence and health improvements comparable to voluntary clients. However, challenges including legal ambiguities, resource limitations, and inter-systemic stigma hinder broader implementation. Despite these barriers, the findings support the utility of court-mandated MMT as a rehabilitative and restorative intervention.
RECOMMENDATIONS
Strengthened policy linkages between judicial and health systems are essential. Standard operating procedures for court-initiated referrals and robust monitoring frameworks are recommended to enhance the effectiveness and scalability of court-directed methadone treatment programs to attain universal health coverage.
